Cannot proceed from Questionnaire Intro:
If you are using a mobile device, you will not be able to proceed, you will need to use an industry standard computer with a modern browser.
If you are using a computer, your installed internet security options can cause the issue that prevents your browser from processing the proceed button. In many cases, simply using another browser is the quickest way to get past the issue.
It has been reported that Chrome in some cases might Throttle Navigation and users have stated that they can proceed by continually clicking the Proceed button to force the browser to proceed.
Depending on the version of internet explorer you may need to run in Compatibility mode.
